Amongst the psychopathological effects induced by stimulants and cocaine there can be a variety of psychotic-like experiences. Some of them can be resolved in a matter of hours or a few days without clinical tratment, others instead can constitute initial symptoms of a primary psychosis. The objective of this paper is to focalise on a series of psychodynamic aspects, detectable by the analysis of relational dynamics brought into play by the person who has used the substances and has a psychotic crisis. These aspects can be used as criteria of a differential diagnosis, to integrate a first assessment of those psychopathological characteristics described in literature, which distinguish a toxic psychosis from a primary psychosis. The diagnostic process is developed by a method of clinical observation and evaluation, for which the essential cognitive tool is formed by the sensibility of the therapist and their capacity to evaluate the quality of the patient's mental reactions to the stimulus provoked by the development of the therapeutic relationship, which includes oneiric activity. This paper proposes, on the basis of many years of research and clinical experience, diagnostic criteria also in dream analisysis as oneiric images are able to reveal hidden thoughts and dynamics which can be pathological, like negations or annullaments, or non pathological intuitions. This analysis can detecte eventual psychopathological nucleus of psychosis hidden by a state of intoxication and consent the implementation of adequate clinical treatment based on greater diagnostic certainty.

Notre travail a porté sur la comparaison statistique de deux groupes d’anorexiques mentaux de moins de 13 ans et de plus de 13 ans, examinés dans un service de pédopsychiatrie, et répondant aux critères du DSM-III. II met en évidence les éléments suivants.

1. 1) L’anorexie mentale est peu fréquente chez les enfants de moins de 13 ans par rapport aux adolescents, puisque ce groupe comprend 14 patients sur un intervalle de 7 ans et demi, alors que le groupe des moins de 13 ans comprend 37 patients pour un intervalle de 2 ans et demi.

2. 2) La proportion de garçons est plus élevée dans le groupe des moins de 13 ans puisque nous retrouvons 5 garçons pour 9 filles, les garçons représentant 35% de ce groupe, et la prédominance féminine est plus nette à la puberté puisque nous comptons 1 seul garçon pour 36 filles, les garçons ne représentant donc que 3%.

3. 3) Les retards de croissance sont marqués chez les jeunes anorexiques. Dans notre série, ils ne touchent que des garçons (3). L’anorexie est apparue respectivement à 10 ans pour le premier et à 11 ans et demi pour les deux autres. Pour deux patients, la taille définitive est normale. Le troisième patient, à 23 ans, a une taille inférieure à - 2 DS. On peut s’interroger sur l’absence de retard de croissance dans notre série chez les jeunes anorexiques de sexe féminin. L’explication, à notre avis, réside dans l’âge généralement élevé (> 10 ans) de nos anorexiques filles lors du début de la maladie. Leur croissance était déjà bien avancée et la puberté sans doute amorcée. A l’opposé, les garçons qui ont eu un arrêt de croissance étaient plus jeunes et tous prépubères au début de l’anorexie.

4. 4) Cliniquement, la symptomatologie est plus bruyante dans le groupe des moins de 13 ans avec:

   • – un état prémorbide fait de troubles du comportement alimentaire dans l’enfance;

   • – une perte de poids plus rapide. Elle est souvent inférieure à 25% du poids antérieur mais la cachexie est marquée, le tissu adipeux étant peu abondant avant la puberté. Dans le groupe des jeunes anorexiques, la perte de poids est en général comprise entre 15 et 20%;

   • – un refus d’hydratation associé au refus de nourriture dans 3 cas sur 14, cette particularité n’a pas été observée dans le groupe plus âgé.

   Ces deux dernières constatations mettent en cause les critères DSM-III de l’anorexie mentale pour les jeunes enfants.

5. 5) En revanche, nous n’avons trouvé aucune différence significative entre les deux groupes concernant:

   • – la prévalence des maladies psychiatriques chez les ascendants et la fratrie: elle est de 17,8% chez les parents du groupe 1, en majeure partie faite de troubles thymiques (16%). Dans la fratrie de nos patients, la pathologie la plus souvent retrouvée est un trouble des conduites alimentaires;

   • – la place de l’anorexique dans la fratrie, le statut social des parents, la situation du foyer parental; l’hyperactivité physique, la survenue de vomissements provoqués, la prise de laxatifs et les plaintes somatiques;

   • – la prévalence de la dépression comme diagnostic associé. Elle est néanmoins élevée dans les deux groupes puisqu’on retrouve 86% d’états dépressifs dans le groupe des moins de 13 ans (12 cas sur 14), 60% d’états dépressifs chez les plus de 13 ans (22 cas sur 37);

   • – la sévérité des stress psychosociaux;

   • – le niveau d’adaptation et de fonctionnement social et scolaire dans l’année écoulée. Dans les difficultés d’adapta tion constatées chez nombre de nos anorexiques, rentrent tout autant les troubles de la relation sociale que des dis torsions cognitives responsables de faibles performances, en dépit d’un surinvestissement de la scolarité et d’efficiences intellectuelles tout à fait satisfaisantes.

6. 6) Nos conclusions ne sont que fragmentaires en matière de devenir de l’anorexie du fait de l’imprécision de cer taines données recueillies, de la durée relativement brève de la catamnèse (les anorexiques du groupe 1 ont générale ment bénéficié d’un suivi plus régulier et prolongé - 4 ans en moyenne -, que ceux du groupe 2: 2 ans er moyenne), et de la faiblesse de l’échantillon des moins de 13 ans.

   Le poids s’est normalisé dans 78,5% des cas dans le groupe 1 et 54% des cas dans le groupe 2. On constate done que sur le plan symptomatique, il n’est pas très difficile de refaire prendre du poids aux anorexiques.

   Le comportement alimentaire est satisfaisant chez 9 sujets du groupe 1 (64%) et 19 sujets du groupe 2 (51%)

   L’apparition ou le retour des règles est observé chez 6 filles sur 9 du groupe 1 (66,6%) et 20 filles sur 36 dt groupe 2 (55,5%).

   L’évaluation de l’état mental de nos sujets (elle concerne 12 sujets du groupe 1 sur 14, 2 n’ayant pas répondi aux questions posées) montre qu’une patiente présente une anorexie mentale chronique avec des épisodes récurrent; de dépression majeure, qu’une autre patiente a fait un épisode de dépression délirante et qu’une troisième peut êtri considérée comme schizophrène. Six sujets souffrent d’une anxiété chronique avec manque de confiance en soi e phobie sociale pour l’un d’entre eux. Les 3 derniers ont un fonctionnement satisfaisant dans tous les domaines Comme on le voit, l’anorexie mentale est une maladie grave dont le pronostic est réservé. Cependant l’évolution ne nous a pas semblé différente dans les deux groupes concernant les paramètres comportementaux. Il faut cepen dant souligner qu’à deux exceptions près, les anorexiques de moins de 13 ans avaient débuté leur maladie immédia tement avant la puberté ou tôt après son début.

Research of the last decade established that inflammatory reactions are involved in the pathogenesis of Alzheimer's disease (AD) and schizophrenia (Sch).

The aim of this paper to carry out a comparative analysis of some inflammation mediators [activity of leukocyte elastase (LE) and alpha 1- proteinase inhibitor (α1-PI), the level of CRP, IL-6 and IL-10] in the blood serum in patients with AD and in old aged patients with schizophrenia in comparison with healthy control of the same age.

There were a significant increase in activity of α1-PI, level of CRP, IL-6 and IL-10 (p<0.0001, p<0.05, p<0.01, p<0.01, respectively) and significant decrease of LE activity (p<0.0001) in the group of patients with AD compared to control. The activity of LE was positive correlated with MMSE scores (r =0.34, ?=0.0487) and the IL-6 level was negative correlated with MMSE scores (r =-0.46, ?=0.0077), ie cognitive decline was associated with decreased activity of LE and increased levels of IL-6. The group of patients with Sch was characterized only by a significant increase in activity α1-PI and level of CRP compared to control (p<0.0001, p<0.01, respectively). LE activity and IL-6 level in patients with AD significantly differ from patients with schizophrenia (p<0.05, p<0.01, respectively).

Thus, the low activity LE and high levels of IL-6 are observed only for patients with AD, but not for patients with schizophrenia. These parameters may serve as biological markers of the severity of AD.

In recent years, with the support of the Stability pact for south-east Europe, and following the example of the EU countries, there has been developed a new concept of mental health care. It should be carried out by newly opened centers for mental health. Goal of the work is to set forth numerous problems which considerably slow down development of these modern aproach to mental health care,as well as solutions to them.

Problems in the realisation of the project, have been stated and classified in the discussion together with explanation of their causes and consequences:

1. - unadjusted or non-existent accompanied legal acts;

2. - negligence of bureaucracy, resistance in other social services;

3. - prejudices and opposition of “official” psychiatry and of profesionals in mental health institutions;

4. - insufficient education and shortage of trained staff

5. - lack of understanding and resistance in a wider community;

6. - financial problems.

Furtheron, methods have been stated for finding “less painful” solutions to these problems. This experience is probably similar to those in all countries in transition and should help that the same mistakes would not be made in further development of communal psychiatry. This work contains recommedations how to prevent and avoid these obstacles and may be useful in those places where developmemnt of such aproach to mental health care just being planned.

The brain mechanisms related to formal thought disorders (FTD) and auditory verbal hallucinations (AVH) appear to be closely linked to structural and functional abnormalities of the language system.

In particular, functional imaging data indicate that several components of the language system are involved in the generation of both, FTD and AVH. Co-activation of the primary auditory cortex (Heschl's gyrus) during verbal thoughts appears to be crucial for the subjective perception of an externally generated voice, and DTI studies indicated that the intrahemispheric fronto-temporal connectivity of the language system is specifically increased in hallucinating patients. On the other hand, FTD are significantly correlated to a gray matter reduction in Wernicke's region along with a hyperactivation of frontal and temporal components of the language system.

AVH are intimately related to a dysfunction of the left hemispheric language system, including the primary auditory cortex and the fronto-temporal fibre tracts connecting Broca's and Wernicke's regions. Co-activation of the primary auditory cortex during verbal thoughts appears to be the basis of the pathological network dynamics during AVH, while FTD are linked to a pathological hyperactivity of central components of the language system. Both phenomena are related to functional imbalances of the language system, and phenomenological differences may depend on the different contributions of the system components.

Few epidemiological studies have explored hallucinations’ prevalence and types in the general population. Ohayon et al. [1] have emphasized the high prevalence of hypnagogic and/or hypnopompic hallucinations present respectively in 37 and 12.5% of the 4972 subjects. The rate of hypnagogic hallucinations was related to age. Women were more likely to report such hallucinations. The most frequent type of hallucinations was unexpected, that is kinesthesic (feeling of falling in an abyss) but all types of hallucinations could be present. These hallucinations had no relationship to a specific pathology in more than 50% of the cases but were exclusively present at nighttime. In old age, visual hallucinations are the most frequent. In a cohort of old non-demented subjects, 17.4% of them had hallucinations and behavioural symptoms (anxious agitation or/and irritability).

Several neurological pathologies included sleep disorders and hallucinations. Narcolepsy and Parkinson disease (P.D.) will be only considered here. In narcolepsy, with or without cataplexy, hallucinations are now called “secondary” symptom, as they are incidental, reported in 35 to 66% of the patients.

Hypnagogic and hypnopompic hallucinations are most often visual but all types have been described, formed, without whole scenes but terrifying the patient who is often implicated in it and sometimes associated with sleep paralysis, especially during daytime when sleep occurs in REM. Are they pieces of dream? These hallucinations can be differentiated on a phenomenological basis from hallucinations in healthy subjects and from hallucinations in schizophrenia [2]. They have also some differences with hallucinations observed in P.D. [3].

In P.D. hallucinations affected 25% of the patients in long duration disease. They are mainly visual, or with sensation of a presence (person or animal), coexisting with delusions and anxiety. They are rarely only a side- effect of dopaminergic treatment but are often linked to daytime sleepiness with REM sleep attacks, described in this disease and to cognitive impairment.

La décision médicale partagée (DMP) est un mode de relation thérapeutique visant à favoriser la participation active du patient à la décision en clarifiant notamment ses valeurs et ses préférences. La pertinence d’une telle approche peut être questionnée en addictologie où il semble que l’enjeu soit moins d’aider le patient à choisir entre des options a priori équivalentes que de promouvoir un changement profitable pour sa santé (typiquement l’abstinence vis-à-vis du produit). Les évolutions récentes de la discipline remettent cependant en cause cette vision des choses : le contrôle de la consommation et la réduction de risque apparaissent de plus en plus comme des objectifs légitimes de la prise en charge et on assiste à une multiplication des options thérapeutiques, pharmacologique ou autres. De fait, beaucoup d’addictologues appliquent déjà probablement certains principes de la DMP dans leur pratique quotidienne. Les travaux spécifiques sur ce sujet restent cependant quasi-inexistants. À titre d’illustration, alors que les travaux publiés sur la DMP se comptent par centaines en cancérologie et par dizaines en psychiatrie, on n’en dénombre à peine cinq en addictologie tous issus de la même équipe. Mon but sera d’offrir une réflexion sur la façon dont la DMP peut s’intégrer aux soins en addictologie et, en particulier, sur sa complémentarité avec l’approche motivationnelle. J’en apporterai également une illustration concrète sous la forme d’un projet d’outil d’aide à la décision pour le choix d’un éventuel traitement pharmacologique de l’alcoolodépendance.

Sleep disorder is a widespread chronic disease. Its treatment and the disease itself causes losses to society, eg treatment cost and cost of sick leaves. On the other hand an appropriate treatment increases patients' quality of life and work productivity. The basic methods for evaluating the costs and benefits of treating sleep disorders are described.

Transpersonal, meaning beyond the ego-boundaries, was first used by Assagioli. After that the term has been used to indicate an attention to the transcendent or spiritual dimension of humanity, both in philosophy and psychology. In psychotherapy the term transpersonal refers also to the exploration of altered state of consciousness with healing purposes. Starting from these premises P.L. Lattuada developed a model called Biotransenergetica. In such a method the attention is directed to the person in his/her wholeness not just to the symptoms. The core concept is the transe, which refers to a process of being in harmonic equilibrium within oneself and with the surrounding. Symptoms are view as allies showing where the process is blocked, so that the transe is disharmonic, causing physical as well as psychological suffering. The therapeutic process is a way to read the process that led to a disharmonious transe and help the client in a self-healing process, which also will lead toward a self-realization. Transpersonal psychology has been criticized by some authors challenging its definitions that link transpersonal psychology to healthy states only. Moreover recalling the spiritual dimension puts at risk the application of transpersonal psychotherapy in a clinical setting, where scientific evidence based approach is requested. However our clinical experience demonstrates that BTE can be applied in a therapeutic context. Aim of this presentation is to show the clinical application of this model describing how disease and symptoms could be revised in terms of harmonic transe and self-realization.
