In this study we investigated 1) the changes in anxiety, depression and denial from admission to discharge in patients admitted to the intensive care unit following an acute myocardial infarction and 2) the effect of smoking habits, time lapsed from the appearance of symptoms to seeking help behavior, presence of a person that motivated the patient to seek help, previous myocardial infarction (MI) and family history of MI, on these changes. The results indicated that 1) the levels of both anxiety and depression increased from admission to discharge, while denial decreased; 2) positive family history of MI was associated with lower difference of denial between admission and discharge.

We investigated the overall prevalence and the differential comorbidity of Diagnostic and Statistical Manual (DSM)-III-R personality disorders in 166 remitted or recovered patients with schizophrenic (n = 102) or unipolar mood disorder (n = 64). Over 60% of both patient groups met the DSM-III-R criteria of at least one DSM-III-R personality disorder as assessed by means of the Structured Clinical Interview for DSM-III-R (SCID-II-R), receiving on average 3.1 personality diagnoses. Neither DSM-III-R categories of personality disorders, nor scores on its three clusters A, B and C, nor total score on SCID-II-R differed significantly across the two groups. In conclusion, DSM-III-R personality disorders, although highly prevalent in schizophrenic and unipolar mood disorders, lack any specificity with respect to these categories of mental disorders.

Six chronic neuroleptic-resistant schizophrenic patients, partial responders to clozapine, were co-administered 600 mg/day of sulpiride (a selective D2 dopaminergic antagonist) as an augmentation to clozapine (a relatively weak D2 blocker), for 10 weeks, open-labeled. A remarkable reduction in positive and negative symptoms was observed in four of the six patients.

The term ‘quality of life’ has only recently been introduced to psychiatric outcome assessment. Current approaches to such assessment include the development of disease-specific quality of life instruments for psychiatric patient populations as well as the examination of generic quality of life instruments for use in psychiatric outcome studies. The current paper describes the psychiatric work with one example for a generic instrument: the short-form health survey questionnaire (SF-36). The 36 item patient-based questionnaire was developed within the Medical Outcome Study in the United States, was translated, validated and standardized in several countries and has been used in psychiatric settings. Results of epidemiological studies suggest that the SF-36 is applicable to psychiatric patients, is a psychometrically sound instrument also in this indication and yields relevant information in showing the degree of impairment in quality of life domains as experienced by psychiatric patients. The use of generic instruments in psychiatric population, such as the SF-36, might contribute to a better understanding of patients' quality of life as assessed in epidemiological studies, clinical trials and quality of care evaluations.

Obtaining a level of consensus over the definition, construction and measurement of the concept of quality of life would allow for an improved degree of standardization in the assessment of clinical intervention for people with mental health problems. One of the many benefits of this standardization would be the ability to make valid and reliable comparisons between various interventions and across different groups or settings, which is of particular interest to economists. There are, however, a host of sociocultural issues that present fundamental obstacles to the satisfactory attainment of consensus over definitions and domains of quality of life. This paper considers the arguments pertinent to each of these two alternative perspectives, the economic and the sociocultural (or anthropological), and draws out the lessons that these perspectives — despite the apparent polarity that exists between them — can offer to the improved measurement of quality of life for those with mental health problems.

We investigated research activities on schizophrenia in 17 non-English speaking (13 European: Austria, Belgium, Denmark, Finland, France, Germany, Greece, Italy, Luxembourg, Netherlands, Portugal, Spain, and Sweden and 4 Asian: China, Japan, South Korea, and Taiwan) countries via a MEDLINE database survey. Despite the handicap of language, a reassuringly large number of English-written papers in research into schizophrenia have been reported; 892 papers from these countries appeared over the period 1990–1994. Among the countries investigated, Scandinavian countries in particular were found to have produced more communicable papers after taking into account the number of psychiatrists.
