Tobacco is a known addictive consumer product and its use has been reported to be associated with several health problems as well as the leading cause of premature, preventable mortality worldwide. For patients undergoing cancer treatment, tobacco smoking can potentially compromise treatment effectiveness; however, there is sufficient evidence suggesting numerous health benefits of smoking cessation interventions for cancer patients.

The Grand River Regional Cancer Centre (GRRCC) smoking cessation program began in October 2013 to provide evidence-based intensive tobacco intervention to patients. All new patients are screened for tobacco use and those identified as active smokers are advised of the benefits of cessation and offered referral to the program where a cessation nurse offers counseling. Patients’ disease site, initial cessation goal, quit date, number of quit attempts and mode of contact are collected by the cessation nurse. This study reports on the initial evaluation of the smoking cessation program activities at GRRCC.

There are 1,210 patients who were screened, accepted a referral and counseled in the program. The referral pattern shows a modest increase every year and most of the patients (58%) indicated readiness to quit smoking. Overall, 29 and 26% of patients either quit or cut-back smoking, respectively. Among 348 patients who quit smoking, 300 (86%) were able to quit at the first attempt. The data indicated that 309 (44%) out of the 698 patients who indicated their initial intent to quit smoking were able to quit, whereas about 242 (35%) were able to cutback. A total of 15 patients out of 32 who indicated initial readiness to ‘cutback’ smoking were able to reduce tobacco use and three patients actually ended up quitting, although their initial goal was ‘ready-to-cut-back’.

GRRCC smoking cessation program started in October 2013 to provide evidence-based intensive smoking cessation interventions for patients with cancer. Most patients referred to the program indicated a readiness to quit smoking affirming that if patients become aware of the various risks associated with continual smoking or if they are informed of the benefits associated with cessation with regard to their treatment, they will be more likely to decide to quit. Therefore, it is essential that patients, their partners and families are counseled on the health and treatment benefits of smoking cessation and sustainable programs should be available to support them to quit smoking. It is imperative then, that oncology programs should consistently identify and document the smoking status of cancer patients and support those who use tobacco at the time of diagnosis to quit. Evidence-based smoking cessation intervention should be sustainably integrated into the cancer care continuum in all oncology programs from prevention of cancer through diagnosis, treatment, survivorship and palliative care.

Varian RapidArc is a volumetric modulated arc therapy (VMAT) that obtains a conformal dose around the desired structure by employing variable gantry speed, dose rate and dynamic multileaf collimator (DMLC) speed as the gantry rotates about machine isocenter. This study is meant to build upon previous research by Ling et al. by completing the tests with an in vivo dosimetric device attached to the linac gantry and a 2D ionisation chamber array with an isocentric gantry mount.

Two PTW detectors, seven29 array with gantry mount and DAVID, were attached to the linear accelerator gantry, allowing each device to remain perpendicular to the beam at all gantry angles. Three tests for RapidArc evaluation were performed on these devices including: dose rate and gantry speed variation, DMLC speed and dose rate variation and DMLC position accuracy. The reproducibility of the arc data was also reported.

A picket fence plan varying dose rates (111 to 600 MU/minute) and gantry speeds (5·5 to 4·3°/second) was delivered consisting of seven sections of different combinations. These measurements were compared with static gantry, open field measurements and found to be within 2·39% for the DAVID device and 0·84% for the seven29. A four-section picket fence of varying DMLC speeds (0·46, 0·92, 1·84 and 2·76 cm/second) was similarly evaluated and found to be within 1·99% and 3·66% for the DAVID and seven29, respectively. For DMLC position accuracy, a picket fence arc plan was compared with a static picket fence and found to agree within 0.38% and 2.91%. Reproducibility for these three RapidArc plans was found to be within 0·30% and 2·70% for the DAVID and seven29.

The DAVID and seven29 detectors were able to perform the RapidArc quality assurance tests efficiently and accurately and the results were reproducible. Periodic verification of DMLC movement, dose rate variation and gantry speed variation relating to RapidArc delivery can be completed in a timelier manner using this equipment.

Globally, cancer is the second leading cause of death, and it is estimated that over 18·1 million new cases are diagnosed annually. The COVID-19 pandemic has significantly impacted almost every aspect of the provision and management of cancer care worldwide. The time-critical nature of COVID-19 diagnosis and the large number of patients requiring hospitalisation necessitated the rerouting of already limited resources available for cancer services and programmes to the care of COVID-19 patients. Furthermore, the stringent social distancing, restricted in-hospital visits and lockdown measures instituted by various governments resulted in the disruption of the oncologic continuum including screening, diagnostic and prevention programmes, treatments and follow-up services as well as research and clinical trial programmes.

We searched several databases from October 2020 to January 2021 for relevant studies published in English between 2020 and 2021 and reporting on the impact of COVID-19 on the cancer care continuum. This narrative review paper describes the impact of the COVID-19 pandemic on the cancer patient care continuum from screening and prevention to treatments and ongoing management of patients.

The COVID-19 pandemic has profoundly impacted cancer care and the management of cancer services and patients. Nevertheless, the oncology healthcare communities worldwide have done phenomenal work with joint and collaborative efforts, utilising best available evidence-based guidelines to continue to give safe and effective treatments for cancer patients while maintaining the safety of patients, healthcare professionals and the general population. Nevertheless, several healthcare centres are now faced with significant challenges with the management of the backlog of screening, diagnosis and treatment cases. It is imperative that governments, leaders of healthcare centres and healthcare professionals take all necessary actions and policies focused on minimising further system-level delays to cancer screening, diagnosis, treatment initiation and clearing of all backlogs cases from the COVID-19 pandemic in order to mitigate the negative impact on cancer outcomes.

Irish healthcare has seen radical reforms in recent years. Regulatory Body Registration was introduced to improve patient care and regulate professionals. Continuing Professional Development (CPD) is a requirement of registration. Professionals need to keep up-to-date with new techniques and technologies while maintaining their skills.

This research assessed the factors affecting CPD participation in Radiation Therapists and Diagnostic Radiographers and their knowledge and attitudes towards Regulatory Body Registration.

Online surveys were designed and made available on the Irish Institute of Radiography and Radiation Therapy (IIRRT) Website for IIRRT members. The responses were analysed using descriptive statistics and χ2 tests in a statistical computer programme.

Personal development, time, cost and staff shortages negatively affect CPD participation. Reflective practise is not a barrier to CPD. Knowledge and attitudes to Regulatory Body Registration varied.

CPD must be developed in Radiation Therapy and Diagnostic Radiography. Funding, time and increased staffing could result in effective implementation of CPD. Regulatory Body Registration has been communicated but more information regarding the process is required.

The unique behaviour of microbubbles under ultrasound acoustic pressure makes them useful agents for drug and gene delivery. Several studies have demonstrated the potential application of microbubbles as a non-invasive, safe and effective technique for targeted delivery of drugs and genes. The drugs can be incorporated into the microbubbles in several different approaches and then carried to the site of interest where it can be released by destruction of the microbubbles using ultrasound to achieve the required therapeutic effect.

The objective of this article is to report on a review of the recent advances of ultrasound-mediated microbubbles as a vehicle for delivering drugs and genes and its potential application for the treatment of cancer.

Ultrasound-mediated microbubble technology has the potential to significantly improve chemotherapy drug delivery to treatment sites with minimal side effects. Moreover, the technology can induce temporary and reversible changes in the permeability of cells and vessels, thereby allowing for drug delivery in a spatially localised region which can improve the efficiency of drugs with poor bioavailability due to their poor absorption, rapid metabolism and rapid systemic elimination.

The aim of this work was to determine if volumetric modulated arc therapy (VMAT) plans, created for constant dose-rate (cdrVMAT) delivery are a viable alternative to step and shoot five-field intensity modulated radiation therapy (IMRT).

The cdrVMAT plans, inverse planned on a treatment planning system with no solution to account for couch top or rails, were created for delivery on a linear accelerator with no variable dose rate control system. A series of five-field IMRT and cdrVMAT plans were created using dual partial arcs (gantry rotating between 260° and 100°) with 4° control points for ten prostate patients with the average rectal constraint incrementally increased. Pareto fronts were compared for the planning target volume homogeneity and average rectal dose between the two techniques for each patient. Also investigated were tumour control probability and normal tissue complication probability values for each technique. The delivery parameters [monitor units (MU) and time] and delivery accuracy of the IMRT and VMAT plans were also compared.

Pareto fronts showed that the dual partial arc plans were superior to the five-field IMRT plans, particularly for the clinically acceptable plans where average rectal doses were less for rotational plans (p = 0·009) with no statistical difference in target homogeneity. The cdrVMAT plans had significantly more MU (p = 0·005) but the average delivery time was significantly less than the IMRT plans by 42%. All clinically acceptable cdrVMAT plans were accurate in their delivery (gamma 99·2 ± 1·1%, 3%3 mm criteria).

Accurate delivery of dual partial arc cdrVMAT avoiding the couch top and rails has been demonstrated.

In this study, the differences between calculated and measured dose values were then analysed to assess the performance, in terms of accuracy, of the tested treatment planning system (TPS) algorithms applied to calculate electron beam dose targeted and non-targeted the breast region.

The beryllium oxide (BeO) dosimeters placed on the female RANDO phantom were irradiated 12 MeV electron energy with medical linear accelerator and repeatedly read in the Risø thermoluminescence (TL)/optically stimulated luminescence (OSL) system via OSL method at least three times.

For electron treatment, one made quantitative comparisons of the dose distributions calculated by TPSs with those from the measurements by OSL at various points in the RANDO phantom.

The mean dose measured from the dosimeters placed on the female RANDO phantom target left breast region was 160 cGy and non-target right breast region was 1·2 cGy. Analysis of Generalised Gaussian Pencil Beam (GGPB) and Electron Monte Carlo (eMC) algorithms for determined region mean point dose values, respectively, 174 and 164 cGy. Two algorithms for non-targeted region calculated same point dose values of 0·2 cGy.

The results of this study showed that BeO dosimeters can be used with OSL method in radiotherapy applications and it is a very important tool for the determination of targeted/non-targeted absorbed dose.

Whole-breast external beam radiotherapy results in significant reduction in the risk for breast cancer-related death, but this may be offset by an increase in deaths from other causes and toxicity to surrounding organs. Partial breast irradiation techniques are approaches that treat only the lumpectomy area rather than the whole breast. Quality assurance in the radiation therapy treatment planning process is essential to ensure accurate dose delivery to the patient. For this purpose, this article compares the results from an anthropomorphic PRESAGE® dosimeter, radiation treatment planning system and from the GAFCHROMIC® EBT2 film.

A breast dosimeter was created and a three-field partial plan was generated in the Pinnacle3 treatment planning system. Dose distribution comparisons were made between Pinnacle3 treatment planning system, GAFCHROMIC® EBT2 film and PRESAGE® dosimeter. Dose–volume histograms (DVHs), gamma maps and line profiles were used to evaluate the comparison.

DVHs of gross tumour volume, clinical tumour volume and planning tumour volume for the PRESAGE® dosimeter and Pinnacle3 treatment planning system shows that both measured and calculated statistics were in agreement, with a value of 97.8% of the prescribed dose. Gamma map comparisons showed that all three distributions passed 95% at the ±3%/±3 mm criteria. Comparisons of isodose line distribution between the PRESAGE® dosimeter, EBT2 film and planning system demonstrated agreement, with an average difference of 1.5%.

This work demonstrated the feasibility of PRESAGE® to function as an anthropomorphic phantom and laid the foundation for research studies in PRESAGE®/optical-computed tomography three-dimensional dosimetry with the most complex anthropomorphic phantoms.

We propose to use the PTV dose–volume factor (PDVF) to evaluate treatment plans of prostate volumetric modulated arc therapy (VMAT) and intensity modulated radiotherapy (IMRT).

PDVF was used to compare the variation of planning target volume (PTV) coverage between VMAT and IMRT because of weight loss of patient.

VMAT and IMRT plans of five patients (prostate volume = 32–86·5 cm3) using the 6 MV photon beams were created with the external contour reduced by depths of 0·5–2 cm to reflect the weight loss. Moreover, integral doses (volume integral of the patient dose) and prostate tumour control probability (TCP) were calculated.

We found that reduced depth resulted in PDVF decreasing 0·03 ± 4·7 × 10−4 (VMAT) and 0·04 ± 9·7 × 10−3 (IMRT) per cm for patients. The decrease of PDVF or degradation of PTV coverage was found more significant in IMRT plans than VMAT with patient size reduction. The integral dose did not change significantly between VMAT and IMRT, while the prostate TCP increased with an increase of reduced depth.

We concluded that PDVF can be successfully used to evaluate the variation of PTV coverage because of weight loss of patient in prostate VMAT and IMRT. Degradation of PTV coverage in prostate VMAT is less significant than IMRT regarding patient size reduction.

Sexual dysfunction is a common side effect of external beam radiotherapy (EBRT) and androgen deprivation therapy (ADT) to treat prostate cancer. Men are likely to experience erectile dysfunction, low libido, ejaculatory problems and penile shortening. This qualitative study explored men’s perceptions of sexual dysfunction, including factors such as self-perception, relationships and information and support needs.

Semi-structured interviews were carried out with n = 8 men living 18–30 months after EBRT ± ADT. The interviews were transcribed and thematic analysis was carried out.

All men experienced sexual dysfunction following treatment. The main themes arising were: (i) priorities—sexual issues were not a priority when making treatment decisions, (ii) information and support—men described a lack of information and support about sexual dysfunction and (iii) impact—sexual dysfunction impacted on their self-perception and relationships.

Men undergoing EBRT/ADT for prostate cancer may be affected by post-treatment changes in sexual function in a range of ways. This study suggests that they would benefit from early and wide-ranging information and support on sexual dysfunction, even if they do not consider it as a priority. Candid discussions about self-perception and relationships, as well as physical changes, may equip them to cope with post-treatment changes.
