This study reveals the characteristic nature and the use of optically stimulated luminescence dosimeters (OSLD) as an in vivo dosimetry tool for head and neck intensity-modulated radiation therapy (IMRT).

Calibration and characterisation of OSLD such as sensitivity, reproducibility, dose-rate dependence, beam quality dependence, output factor measurement and comparison of two bleaching techniques using halogen and compact fluorescent lamp (CFL) were initially performed. Later, eye dose measurements were performed for head and neck IMRT patients using OSLD and were compared with the corresponding dose calculated by the treatment planning system (TPS).

While the sensitivity was found to be within ±5%, the dose-rate dependence and reproducibility were found to be within ±3%. The OSLD showed an under-response of 3% for 15 MV and an increase in response by 5% for Co60 (1·25 MeV) when compared with the 6 MV X-ray beam. Therefore, a separate calibration for different beam energies is required. The percentage deviation of OSLD to that of TPS was found to be within ±2·77%. The OSLD has been successfully used for the in vivo dosimetry of patients who received IMRT. Hence, it is concluded that OSLDs can serve as effective dosimeters for in vivo dosimetry.

Emotional intelligence (EI) is an increasingly important aspect of a health professional’s skill set. It is strongly associated with empathy, reflection and resilience; all key aspects of radiotherapy practice. Previous work in other disciplines has formed contradictory conclusions concerning development of EI over time. This study aimed to determine the extent to which EI can develop during a radiotherapy undergraduate course and identify factors affecting this.

This study used anonymous coded Likert-style surveys to gather longitudinal data from radiotherapy students relating to a range of self-perceived EI traits during their 3-year degree. Data were gathered at various points throughout the course from the whole cohort.

A total of 26 students provided data with 14 completing the full series of datasets. There was a 17·2% increase in self-reported EI score with a p-value<0·0001. Social awareness and relationship skills exhibited the greatest increase in scores compared with self-awareness. Variance of scores decreased over time; there was a reduced change in EI for mature students who tended to have higher initial scores. EI increase was most evident immediately after clinical placements.

Radiotherapy students increase their EI scores during a 3-year course. Students reported higher levels of EI immediately after their clinical placement; radiotherapy curricula should seek to maximise on these learning opportunities.

To determine the frequency, factors and reasons of patient non-adherence to radiotherapy (RT) in a tertiary cancer centre.

Inadvertent treatment interruptions often lead to prolongation of planned treatment time. In the case of RT with a curative intent, prolongation of planned treatment has been associated with inferior clinical outcomes. Delay or prolongation of treatment is associated with a relative risk of local recurrence by up to 2% per day for specific malignancies. Thus, it is critical to understand key factors that influence non-adherence to RT.

A retrospective observation audit was conducted comprising patients treated with radical, adjuvant or palliative RT at our centre from January 2018 to December 2018. Non-adherence was defined as premature permanent termination of planned treatment by the patient without recommendation or consultation from the treating clinician. All data were collected and analysed (retrospectively) with the help of Statistical Package for the Social Sciences (SPSS) version 22.

A total of 1,548 patients were included in the study of which 105 (6·7%) were non-adherent to planned RT. Of the total 105 patients, 44 (42%) were elderly (60 years and above). Treatment non-adherence was predominant in males (male:female = 1·85:1). More than 90% of non-adherent patients had stage III and IV cancer. A total of 77 patients (74%) out of 105 were more than 50 km away from our centre. A total of 66 (63%) out of 105 patients had completed more than 2 weeks of radiation (40% of planned RT) and then defaulted for radiation due to acute toxicities.

Treatment adherence is a major factor in determining successful outcomes among cancer patients treated with RT. This study reveals several factors that contribute to non-adherence to treatment.

Achieving daily consistent bladder volume is acknowledged as challenging for patients undergoing radiotherapy to the cervix or endometrium. We investigated if use of an ultrasound bladder volume scanner (BioCon-700) improves bladder reproducibility when used during an active volume correction protocol.

During our method-comparison study, prospectively recruited patients (n=20) followed a fluid-loading protocol to achieve acceptable bladder volume. Bladder ultrasound was performed daily to verify planned volume, with patients actively correcting volumes outside a planned range up to a maximum of three times. Using the Bland–Altman method, we compared mean ultrasound readings (USMean) with mean cone-beam computed tomography (CBCT) volumes (CBCTMean). We also conducted staff focus groups exploring issues encountered during implementation of bladder scanning.

Comparing USMean with CBCTMean produced a mean of the differences −10±49·92 mL (1 SD), demonstrating that bladder volume scanning is equivalent to our standard measure for the stated confidence levels. The cohort mean bladder volume decrease from week 1 to 5 was only 8·4%. Mean USMean was 323 mL, mean CBCTMean was 313 mL. Staff experience with the scanner overall was positive.

The BioCon-700 is suitable for the purpose of daily pre-treatment volume verification, facilitating daily assessment and modification of bladder volume, resulting in reproducible treatment volumes.

The Coronavirus disease 2019 (COVID-19) is spreading rapidly throughout the world. Lung is the primary organ which the COVID-19 virus affects and leads to pneumonia, an acute respiratory distress syndrome. COVID-19 infects the lower respiratory system, and the lung’s response to this infection is recruiting macrophages and monocytes leading to inflammation, this response causes widespread damage to the lung’s airways.

The purpose of this study is to review studies of using low-dose radiation as a treatment for the inflammation of the tissue and pneumonia resulting from COVID-19. These studies were compared with the risk of developing lung cancer during performed dose for the treatment of COVID-19 in radiation therapy.

Our study focused on in vitro, in vivo and clinical reports of using low-dose radiation for the treatment of inflammation, pneumonia and COVID-19. The risk of lung cancer resulting from suggested dose in these studies was also evaluated.

From the review of articles, we have found that low-dose radiation can lead to improvement in inflammation in different line cells and animals; in addition, it has been effective in treating inflammation and pneumonia caused by COVID-19 in human up to 80%. Since suggested doses do not remarkably increase the lung cancer risk, low-dose radiation can be an adjuvant treatment for COVID-19 patients.

To identify treatment outcome, dose uniformity, treatment time, toxicity among 3D conformal therapy (3D-CRT), intensity-modulated radiation therapy (IMRT), volumetric-modulated arc therapy (VMAT) for non-small-cell lung cancer (NSCLC) based on literature review.

A literature search was conducted using PubMed/MEDLINE, BMC—part of Springer Nature, Google Scholar and iMEDPub Ltd with the following keywords for filtering: 3D-CRT, IMRT, VMAT, lung cancer, local control and radiobiology. A total of 14 publications were finally selected for the comparison of 3D-CRT, IMRT and VMAT to determine which technique is superior or inferior among these three.

Compared to 3D-CRT, IMRT delivers more precise treatment, has better conformal dose coverage to planning target volume (PTV) that covers gross tumour with microscopic extension, respiratory tumour motion and setup margin. 3D-CRT has large number of limitations: low overall survival (OS), large toxicity, secondary malignancies.

It is difficult to choose the best technique for treating NSCLC due to patient conditions and technique availability. A high-precision treatment may improve tumour control probability (TCP) and patient’s quality of life. VMAT, whether superior or not, needs more clinical trials to treat NSCLC and requires longer dose optimisation time with the greatest benefit of rapid treatment delivery, improved patient comfort, reduced intrafraction motion and increased patient throughput compared to IMRT and 3D-CRT.

The error in set-up of patients is an inherent part of treatment processes. The positioning errors can be used to determine the margins of the planning target volume (PTV) to cover the target volume, while minimising the radiation dose delivered to normal tissues. This study aimed to evaluate random and systematic errors occurring in inter-fraction set-ups of pelvic radiotherapy measured by electronic portal imaging device (EPID) and then to propose the optimum clinical target volume (CTV) to PTV margin in pelvic cancer patients.

This study examined 22 patients treated with pelvic radiotherapy. A total of 182 portal images were evaluated. Population random (σ) and systematic (Σ) errors were determined based on the portal images in three directions (X, Y and Z). The set-up margin for CTV to PTV was calculated by published margin formulae of International Commission on Radiation Units and measurements (ICRU) report No. 62 recommendation and formulas presented by Stroom and Heijmen and Van Herk et al.

Systematic set-up errors for radiotherapy to patients ranged between 2·36 and 4·99 mm, and random errors ranged between 1·51 and 2·74 mm. The margin required to cover the target volume retrospectively was calculated based on ICRU 62 and formulas presented by Stroom and Heijmen and Van Herk et al. were used to calculate the range 2·8–5·7 mm, 5·7–11·9 mm and 6·9–14·4 mm, respectively.

According to our findings, it can be concluded that by extending the CTV margin by 6·9–14·4 mm, we can ensure that 90% of the pelvic cancer patients will receive ≥ 95% of the prescribed dose in the CTV area.

Across the history of radiotherapy, with gradual technological progress and various methods of irradiation, the purpose has always been to deliver homogeneously 100% of the prescribed dose to 100% of the target volume containing the identifiable tumour and/or tumour cells potentially present while limiting the dose to adjacent normal tissues.

The formula for triple point conformity scale is: CS3=(V95+V100+V105)/3VT. (a) Lower limit determination: CS3=(VT+0·93 VT+0·0)/3VT=0·643; (b) Upper limit determination: in order to find out an empirical relation in between V105 and VT, we studied over 593 cancer patients of various sites by taking planning target volume as target, and an empirical relation is derived out as: V105/VT=0·0007. Hence, CS3=(VT+VT+0·0007 VT)/3VT=0·6667~0·667.

Upper and lower limits of CS3 have been calculated at 0·643 and 0·667, respectively. Maximum value of CS3 index is recorded 0·656 while minimum value is 0·478.

The CS3 scale constitutes an attractive tool because it could facilitate decisions during analysis of various treatment plans proposed for conformal radiotherapy. Its major advantages are its simplicity and integration of multiple parameters.

The triple point conformity scale (CS3) provides better qualitative information about radiotherapy plans as compared to other conformity indices. This study advises the users to use the CS3 scale to evaluate a conformal radiotherapy plan which encompasses a wide range of relevant clinical volumes, and is able to extract qualitative dosimetric information.

Geometric uncertainties limit the accuracy of three-dimensional conformal radiotherapy treatments. This study aims to evaluate typical random and systematic set-up errors and analyse the impact of no action level (NAL) correction protocol on systematic set-up errors and clinical target volume (CTV)–planning target volume (PTV) margins.

A total 668 pairs of orthogonal electronic portal images were compared with digitally reconstructed radiographs from computed tomography planning scans for 100 patients consecutively treated during 2011. Patients were divided into groups depending on the treated anatomical region. Patient-specific and population random and systematic set-up errors were calculated. Impact of application of NAL correction protocol on systematic set-up errors and CTV–PTV expansions were evaluated.

Population set-up errors resulted from about 1 mm in head and neck to 2–3 mm in prostate, rectum, lung, breast and gynaecological districts. Patient-specific systematic set-up errors were higher for breast and gynaecological districts and application of NAL correction protocol gave significant reductions, even higher than 30%. Calculated CTV–PTV margins ranged from 10 mm on left–right direction for prostate to 20 mm on superior–inferior direction for lung.

Set-up errors resulted reasonably controlled and application of NAL correction protocol could further improve the level of accuracy. However, the NAL application alone did not seem to add any substantial benefit on CTV–PTV total margins without the adoption of corrective strategies to reduce other important uncertainties limiting accuracy of three-dimensional conformal radiotherapy.

Bladder complications may be seen in up to 12% of patients treated with pelvic irradiation. To report the long-term efficacy of hyperbaric oxygen therapy (HBOT) on radiation-induced cystitis.

We followed 70 patients diagnosed with radiation cystitis submitted to HBOT in our institution from 2007 to 2013. All patients answered a questionnaire documenting symptom severity pre-HBOT and at the end of the follow-up period using the Late Effects of Normal Tissues – Subjective, Objective, Management, Analytical (LENT-SOMA) scale. Our treatment protocol consisted of multiple sessions (median of 40) of 100% oxygen delivery in a multiplace hyperbaric chamber at 2·4 atm for 80 minutes.

With a median follow-up of 55·5 (4–85) months, the success rate after the follow-up period in terms of haematuria resolution or improvement was 91·4%. The mean score of each subjective variable of LENT-SOMA scale (dysuria, frequency, haematuria, incontinence) were significantly lower after the follow-up period (p<0·05) with the exception of decreased stream (p=0·14). The sum of all subjective scores of LENT-SOMA scale was also significantly lower after the follow-up period (p<0·05). No serious complications were observed.

Our results highlight the safety and long-term benefits of HBOT on haemorrhagic radiation cystitis and other distressful bladder symptoms.

Studying the use of Aperture Shape Controller (ASC) and Convergence Mode (CM) in Eclipse (Varian Medical System) in terms of plan quality and complexity of volumetric modulated arc therapy (VMAT).

Forty VMAT plans were re-optimised for the prostate, prostate + lymph nodes, breast and head & neck patients retrospectively, changing the ASC settings (off, moderate, very high) and CM settings (off, on and extended).

Using ‘on’ or ‘extended’ CM increased plan quality in terms of planning target volume homogeneity and low-dose spread to the organs at risk (OAR). ‘Extended’ CM increased the optimisation time 4·3-fold compared to ‘on’, and deteriorated the plan quality in several simple planning cases. ‘Moderate’ ASC decreased plan complexity with minor effect on plan quality compared to ‘off’, but ‘very high’ ASC had larger adverse dosimetric effects. However, the ASC decreased the plan complexity only if the CM was turned ‘on’.

Using ‘on’ CM increases the plan quality but using ‘extended’ CM is not recommended. The ‘moderate’ ASC decreased complexity without significant adverse effects on plan quality, and even ‘very high’ ASC may be used when plan simplicity is prioritised. However, if CM is not used, the ASC should also be turned off.
