Évaluer l’efficience du palmitate de palipéridone (PP) par rapport aux antipsychotiques les plus communément utilisés en France.

Un modèle médico-économique a été développé afin de simuler la progression d’une cohorte de patients atteints de schizophrénie à travers quatre états de santé (« Stable-traité », « Stable-non-traité », « En-rechute », « Décédé »). PP a été comparé à rispéridone injectable à libération prolongée (ILP), aripiprazole ILP, olanzapine ILP, halopéridol décanoate et olanzapine orale (OO). Les coûts, les années de vie pondérées par la qualité de vie (« Quality-adjusted-life-year » ; QALY) et le nombre de rechutes ont été estimés sur cinq ans selon une perspective tous payeurs. Supposés stabilisés suite à une décompensation clinique, les patients initiaient un antipsychotique et passaient en phase de prévention de la rechute en cas de succès après trois mois. Ils/elles pouvaient arrêter leur traitement après une rechute, un manque de tolérance ou par choix, et passer sur la ligne de traitement suivante jusqu’à la troisième ligne (c.-à.-d. clozapine). Afin de prendre en compte l’observance, les probabilités de rechute en phase de prévention ont été calculées à partir de taux d’hospitalisation sur des données françaises en vie réelle. Les données de tolérance et d’utilité ont été dérivées d’études internationales, et les coûts de sources françaises. La robustesse des résultats a été testée via des analyses de sensibilité.

À 5 ans, PP est le moins coûteux des ILP et est associé à un surcoût de 249 € par rapport à OO. Rispéridone ILP et PP sont associés aux plus grands nombres de QALY. PP domine tous les autres ILP en termes de rechute évitée hormis olanzapine ILP.

PP est le moins coûteux des antipsychotiques ILP en France. OO est l’antipsychotique le moins coûteux, mais est associée à un nombre plus faible de QALY gagnées et de rechutes évitées comparé aux antipsychotiques ILP.

In studies of adult patients with attention deficit hyperactivity disorder (ADHD) atomoxetine (ATX) has demonstrated substantial improvements in ADHD symptomatology using Conners’ Adult ADHD Rating Scales (CAARS). The pattern suggests incremental response over time with no clear plateau of response.

To identify patterns of response to ATX in adult ADHD patients and to describe those trajectories over time

To determine if patients have distinct response trajectories using CAARS in two populations, short term (12 weeks) and long term (24 weeks) treatment data.

Data from 2502 ATX patients, who had an investigator-rated CAARS total score at ≥ short or long term time point, The numbers of trajectory clusters for short term (n=2502) and long term (n=1139) data were identified using hierarchical clustering methods. Linear mixed modelling was used to describe those different trajectories over time.

Using CAARS total, 4 trajectory clusters were identified in short term treated patients and 5 in long term. Three out of 4 short term trajectory clusters (representing 84% of patients) and 4 out 5 long term (representing 96%) showed more successful trajectories. In general clusters with less improvement were those with the worst baseline CAARS and minimal initial improvement. Distinct trajectory patterns of response were found that were incremental over time in all clusters.

Adult ADHD patients receiving atomoxetine have individual trajectories of response that can be divided into 4 short term trajectories and 5 long term trajectories. Further analysis is ongoing to describe these cohorts.

Well-conducted treatment trials and data on predictors of treatment response are both lacking for ADWA, despite the high prevalence of the condition. While benzodiazepines are frequently prescribed for anxiety disorders, etifoxine is also an effective agent not associated with dependence. A randomized controlled trial of etifoxine versus alprazolam for ADWA was undertaken, aiming at comparing efficacy and safety of the two treatments, and investigating predictors of the clinical response.

Two hundred and two adult outpatients with ADWA were enrolled at primary care settings sites in South Africa. Patients were followed for 4 weeks of treatment, and for an additional week after treatment discontinuation. The primary outcome measure was the Hamilton Anxiety Rating Scale (HAM-A) and was assessed by using a non-inferiority analysis. Secondary outcomes included the Sheehan Disability Scale (SDS) and the Clinical Global Impressions-Change Scale (CGI-C). Predictors of response were investigated with a multivariate logistic regression.

Both etifoxine and alprazolam dramatically decreased the HAM-A score at 4 weeks. The difference between treatment groups in HAM-A was of 1.78 [90% CI; 0.23, 3.33] in favor of alprazolam. The two treatments were equally efficacious in patients with more severe anxiety symptoms at baseline. Between week 4 and week 5, after treatment discontinuation, HAM-A scores continued to improve in the etifoxine group, but increased in the alprazolam group (P = 0.019); no difference was found between the two groups regarding the secondary outcomes. No significant predictors of treatment response were found. Patients treated with alprazolam experienced more treatment-related adverse events, mostly of central nervous origin, and especially after medication discontinuation.

This trial confirms the efficacy and safety of etifoxine in the management of ADWA.

The analysis of the clinical outcome of a sample of subjects with schizophrenia, after 25 years of evolution.

. Highlighting some clinical and socio-demographic aspects that might play a role in the heterogeneity of the actual clinical status.

The study sample includes 50 subjects, admitted for the first time in the Psychiatric Clinic of Timisoara during 1985–1987, who were diagnosed with schizophrenia at onset and currently have the same diagnosis (2012). The following parameters were analysed: sociodemographic (gender, age at onset, education, family and professional status) and clinical (type of onset, symptomatology, number of admissions).

The onset was insidious in the majority of cases. There were statistically significant differences between the types of symptoms at onset and the actual clinical status.

The clinical outcome in schizophrenia can vary significantly among patients, even if the diagnosis remains stable over time.

Liaison psychiatry specializes in the interface between physical and psychological health. This discipline to which a large literature was devoted, has developed in recent decades. In our country, because of economic and social factors, the implantation of this ‘sub-specialty’ is still difficult. Its location in general hospitals must go first by its acceptance by the health professionals.

Assess the perception of the management of somato-psychiatric comorbidities in somatic medicine and the practice of liaison psychiatry.

It is a transversal and descriptive study of 68 non-psychiatric physicians practicing in public institutions. They all received self-assessment questionnaires with 15 questions via internet.

The surveyed physicians were mostly medical residents (43%). The sex ratio (M / F) was 0.2. Among participants, 71% had sometimes suspected or diagnosed psychiatric illness among their patients. Depression was the most frequent diagnostic (27% of cases).

The majority of participants (60%) rarely search psychiatric complications of severe somatic diseases. Faced with psychiatric symptoms, 82% of practitioners consistently directed the patient to psychiatric consultation. Finally, 87% of physicians considered the existence of a psychiatric unit in the hospital as beneficial and 58% of them judged the presence of a psychiatrist in the team as crucial.

The results of this study emphasize the importance of collaboration between psychiatry and other medical specialties and the necessity to promote the liaison psychiatry in our country.

Catechol-O-methyltransferase (COMT) is candidate gene for schizophrenia, as it encodes enzyme involved in dopamine and noradrenaline methabolism. However, data concerning association of COMT polymorphism with schizophrenia onset in different ethnic groups are controversial. Impaired acoustic startle prepulse inhibition (PPI) is potential hallmark of vulnerability to schizophrenia, but a few investigations of COMT polymorphism effects on PPI revealed controversial results (Liu et al., 2013).

To estimate effects of COMT polymorphism on PPI and blood monoamines level in Russian populations of schizophrenia patients and healthy persons.

(males 22–55 years old): 39 schizophrenia patients and 32 healthy persons. PPI was estimated according to standard protocol (Calkins et al., 2007). Blood monoamnes estimated using HPLC with electrochemical detection. COMT polymorphism was detected by real-time PCR.

There were significantly (p<0,05) lower heterozygotes frequency, impaired PPI, increased baseline startle latency, decreased plasma DOPAC in patients relatively to controls. Group x Genotype interaction was revealed for PPI and startle latency. In controls VAL/VAL carriers displayed the lowest PPI. In patients the lowest PPI showed VAL/ME T and the longest startle latency - MET/MET carriers. In controls PPI positively correlated with dopamine turnover ratio whereas in patients - with plasma adrenaline level.

In Russian population effects of COMT VAL158/MET polymorphism on PPI significantly differ at healthy and schizophrenia persons. One can propose that PPI level in healthy subjects more strictly depends on dopamine turnover than in schizophrenia patients. Further investigations are needed to clear this problem.

L’utilisation prolongée et les associations de benzodiazépines (BZD) anxiolytiques et hypnotiques exposent à des risques à court et long terme (dépendance, démence, troubles psychomoteurs…). Selon la Haute Autorité de santé (HAS), il n’y a pas lieu d’associer une BZD et un apparenté (zopiclone ou zolpidem) le soir.

– Évaluer les habitudes de prescription des BZD et de leurs apparentés hypnotiques dans une population de patients suivis en psychiatrie hospitalière.

– Suivre sur 6 années l’évolution de ces pratiques de prescription et l’émergence d’alternatives thérapeutiques aux BZD.

– Établir un parallèle avec les recommandations et les actualités de la littérature au sujet de ces risques pendant cette même période.

L’étude rétrospective a été réalisée au centre hospitalier Henri-Laborit (Poitiers) en sélectionnant les ordonnances informatisées comportant des BZD et/ou apparentés sur une période allant du 1er janvier 2008 au 31 décembre 2013, par tranche d’une année. Les associations de ces molécules et leurs posologies ont été répertoriées.

L’analyse de 6511 ordonnances a notamment mis en évidence que la prescription de zolpidem ou zopiclone seuls, sans association à une benzodiazépine, est majoritaire (77,5 % des ordonnances en moyenne) jusqu’en 2010. Puis elle diminue fortement (plus que 38 % en 2013) et elle est inférieure à celle de benzodiazépine seule pendant les 3 années suivantes. Parallèlement, le nombre total d’ordonnances dans cet hôpital est en constante augmentation. L’association de benzodiazépine et d’apparentés sur une même ordonnance reste peu courante, dans 2 % des prescriptions en moyenne, mais la prise des deux se situe le soir dans 91 % des cas (69–100 %).

L’étude montre une diminution de prescription d’hypnotiques apparentés aux BZD, allant de pair avec les mises en garde sur leurs effets indésirables et aux actions de l’HAS. Leur association en soirée à des BZD reste présente et une étude prospective auprès des prescripteurs pour connaître leur choix d’alternative thérapeutique est nécessaire.

Mixed states are characterized by the coexistence of manic and depressive symptoms in the same mood episode. In clinical practice, identification of such states is essential.

The study's purpose was to evaluate our clinical practice for the treatment of mixed access and compare with international recommendations.

This is a retrospective study of the records of patients with bipolar disorder, mixed access hospitalized in our department from August 2009 to January 2012.

The sex ratio was 0.52 (23 men / 22 women.). The average age was 39.4 years. In the majority of patients, treatment of mixed episode was based on a combination of drugs. Prescribed combinations were: Conventional neuroleptic (haloperidol) and antiepileptic (valoproique acid) in 44% of cases, haloperidol and antiepileptic (carbamazepine) in 24%, Atypical antipsychotic (Olanzapine) and antiepileptic (valoproique acid) in 16% of cases.- Atypical antipsychotic (risperidone) and valoproique acid in 8% - olanzapine was prescribed as monotherapy In 8% cases

Our therapeutic conduct is in compliance with the international recommendations.

The occurrence of mixed episodes is often associated with poor response to treatment, greater recurrence of episodes and a higher risk of suicide. The recognition of these conditions and the use of appropriate treatment are needed to improve the prognosis of the disease.

Internet addiction is an emerging disorder, considered as a form of behavioral addiction.

The purpose of this study was to evaluate internet addiction among medical students, undergraduates.

Our study was cross-sectional and descriptive, interesting a sample of 60 students, undergraduate medical studies at the Faculty of Medicine of Sfax- Tunisia.

We performed self-award of:

1. - Questionnaire YIAT: Young's Internet Addiction Test (8 items). A score> = 5 was considered pathological

2. - And a predetermined questionnaire assessing the frequency and reasons for the use of the social network site Facebook.

Among the sample, 23.6% was diagnosed as Internet addicts. The prevalence of Internet addiction did not vary with gender. Consumption of tobacco or alcohol was found associated with 11% of students.

As for the use of the social network Facebook, 20% of students perceive their connection as excessive and 36.4% hold back hard not to see their facebook account. The use of facebook as a mean of entertainment and information on recent events were the most frequently reasons found in respectively 58.2% and 87.3% of cases.

A correlation was found between internet addiction and the perception of the student to have an excessive use of facebook (p =0.02).

Our results are consistent with the literature data on the prevalence of internet addiction. This relatively high rate leads to pay more attention to this particular type of addictive behavior, particularly that associated with other psychopathologic problems, especially social anxiety, which should be sought.

En France, la consommation de psychotropes est élevée, en particulier chez les personnes âgées. Personnes âgées QUID (« PAQUID ») est une étude de cohorte menée sur 3777 personnes âgées de plus de 65 ans et vivant à leur domicile, en Gironde ou en Dordogne, depuis 1988. Durant la période de l’étude, des guides de bonne pratique et des alertes ont été publiés par les agences gouvernementales, afin de favoriser, encadrer et optimiser la prescription des médicaments psychotropes, notamment chez les personnes âgées. L’objectif de cette étude est d’évaluer si les caractéristiques associées à la consommation de médicaments psychotropes ont changé entre la période 1988–1998 et la période 2001–2008 chez les sujets âgés de 75 ans et plus.

Les données analysées sont issues de la cohorte PAQUID menée sur 3777 sujets de plus de 65 ans. Des analyses par régression logistique multivariée ont été utilisées pour identifier les facteurs explicatifs de l’usage de psychotropes, globalement et pour chaque classe, sur les deux périodes.

Sur les deux périodes, plus de la moitié des sujets consommaient des psychotropes. La consommation d’antidépresseurs a plus que doublé entre la première et la seconde période. Être une femme, présenter une démence ou une dépression, être dépendant pour les activités quotidiennes, consommer plus de neufs médicaments augmentent la probabilité de consommer des psychotropes.

La consommation de psychotropes semble augmenter entre les deux périodes. On observe quelques changements en ce qui concerne la consommation de psychotropes globalement et les consommations des classes plus spécifiques. Ces changements peuvent être en lien avec des recommandations des autorités de santé sur le traitement de la dépression et l’arrivée sur le marché de nouveaux produits. D’autres semblent moins explicables.

Les services d’urgences des hôpitaux généraux sont devenus au fil des années un des lieux du soin médico-psychologique en France [1]. En effet, près de 180 000 tentatives de suicides y sont prises en charge chaque année [2]. Il s’agit souvent pour ces patients suicidants du premier contact avec les soins psychiatriques. Les objectifs de notre travail étaient d’étudier le souvenir de cette Prise en charge Médico-Psychologique (PMP) initiale et sa relation avec la qualité de l’adhésion à la PMP ultérieure.

Les suicidants ayant réalisé une IMV étaient inclus dans une étude prospective descriptive réalisée aux urgences du CHU de Poitiers au printemps 2013. L’évaluation du souvenir était recueillie par contact téléphonique entre le 7e et le 12e jour suivant l’IMV. L’évaluation de l’adhésion à la PMP ultérieure était réalisée par contact téléphonique avec le référent des soins psychiques du patient à 2 mois de l’IMV, sur des critères d’alliance thérapeutique et d’observance médicamenteuse.

Trente-sept patients ont été inclus, en majorité des femmes (60 %). L’âge médian était de 36 ans. Plus d’un quart présentaient une altération ou une absence de souvenir de la PMP réalisée aux urgences après leur IMV. Cette altération était reliée à l’existence d’une intentionnalité suicidaire (p < 0,02) et à une surveillance par monitorage (p < 0,007), témoin du caractère de gravité de l’IMV. Près de la moitié des patients présentaient une faible adhésion à la PMP ultérieure. L’adhésion à la PMP ultérieure n’était pas reliée à la qualité du souvenir de la PMP réalisée aux urgences.

L’altération ou l’absence de souvenir de la PMP réalisée aux urgences après une IMV pose la question du délai à respecter entre le geste suicidaire et l’intervention initiale de l’équipe psychiatrique. Par ailleurs, l’absence de relation entre le souvenir de la PMP réalisée aux urgences et l’adhésion à la PMP ultérieure nous incite à rechercher les autres leviers de l’adhésion aux soins chez les patients suicidants.

Internet-based randomized controlled-trial is an emergent design that could be particularly pertinent in problem and pathologic online gamblers.

The proposed design of this first online randomized controlled trial among moderate risk problem and pathologic online poker gamblers with a naturalistic recruitment on the online poker operator website Winamax.

The first part of the study is a cross sectional phase, aiming to identify a screening tool of moderate risk problem and pathologic gambling among routine registered-by- operator data by comparison to the clinical diagnosis tool ‘Canadian Pathological Gambling Index’ (CPGI). The second part is an interventional phase, aiming to assess efficacy of 3 internet-based brief interventions on problem gamblers at 6 and 12 weeks compared.

The cross sectional phase will be proposed systematically to all online problem poker gamblers. The interventional phase will be proposed to all online problem poker gamblers with a CPGI score ≥ 5. Subjects will be randomized in 4 groups: 1. Control= waiting list; 2. email with personal normative information ; 3. self-help program to be downloaded (based on Cognitive Behavioral Therapy and Motivational Interview) ; 4. Same program provided by weekly email contacts with a psychologist. The first judgment criterion will be the decrease in CPGI at six weeks.

The perspectives of this study are to enhance problem and pathologic online gamblers profiling by operators by proposing a surrogate endpoint of problem and pathologic gambling and to propose an easy, acceptable, minimal-efficient online behavioral intervention to problem and pathologic gamblers.

Illustrer la potentialité antidépressive de la kétamine par un cas clinique rapporté.

Femme de 50 ans souffrant d’une dépression secondaire à des algies chroniques lombosciatiques rebelles et invalidantes au niveau fonctionnelle post-chirurgie d’une hernie discale. La patiente se déplaçait en fauteuil roulant et avait un périmètre de marche avec une canne de quelques mètres seulement.

Traitement initial par sulfate de morphine plus antidépresseurs (duloxétine seule, puis association avec mirtazapine) avec amélioration très partielle sur la dépression et sans amélioration au niveau fonctionnel.

La patiente a bénéficié dans le même temps d’une prise en charge en algologie et en rééducation fonctionnelle avec là encore peu ou pas d’amélioration.

La patiente débute un traitement antalgique par kétamine IV (40 mg/j) sous forme de cure de 5 jours toutes les 6 semaines en hôpital de jour, toujours associé à un traitement antidépresseur par duloxétine 120 mg/j avec une amélioration rapide et spectaculaire aussi bien au niveau des symptômes dépressifs qu’au niveau des algies lombosciatiques. Amélioration également au niveau fonctionnel avec disparition de l’aide par fauteuil roulant et reprise d’une marche sans canne avec un périmètre de presque 1 kilomètre.

Poursuite actuel du suivi conjoint psychiatre, algologue avec rémission de la symptomatologie dépressive et poursuite de l’évolution favorable au niveau de la symptomatologie fonctionnelle.
