Afin d'évaluer la fréquence et les manifestations de la dépression à l'adolescence, une enquête a été réalisée auprès de 744 lycéens (439 garçons et 305 filles), âgés de 14 à 23 ans, et appartenant à 15 établissements d'enseignement du second degré du département du Nord. La première partie de l'enquête a consisté en la passation de deux autoquestionnaires: la version française de l'échelle CES-D (Center for Epidemiologic Studies-Depression Scale); et un questionnaire destiné à recueillir un certain nombre de renseignements concernant la situation sociodémographique du sujet, son état de santé et celui de ses parents, son mode de vie, ses relations familiales, son degré d'insertion scolaire et sociale. La deuxième partie a consisté en un examen clinique semi-standardisé visant à repérer les adolescents présentant un épisode dépressif majeur selon les critères diagnostiques du DSMIII-R. Sur les 728 lycéens examinés, 32 (18 garçons et 14 filles) présentaient un épisode dépressif majeur (soit une prévalence de 4,4%). Les critères diagnostiques du DSMIII-R les plus discriminants pour l'identification des adolescents déprimés ont été, par ordre d'importance décroissante: l'humeur dépressive, la diminution de l'intérêt ou du plaisir, l'agitation ou le ralentissement psychomoteur, la diminution de la capacité à réfléchir ou à se concentrer, et les idées récurrentes de mort. Le score moyen obtenu à la CES-D chez les adolescents déprimés apparaît très significativement supérieur à celui obtenu chez les adolescents non déprimés (28,9 ±8 vs 13,5 ± 8,2). Enfin, parmi les variables étudiées, certaines apparaissent significativement associées à la dépression; difficultés scolaires, problèmes de santé multiples et variés, attitudes particulières vis-à-vis du poids et de l'alimentation, problèmes de sommeil, conduites antisociales. Des problèmes de santé, en particulier d'ordre psychiatrique, sont aussi plus fréquemment retrouvés chez les parents des adolescents déprimés. En conclusion, cette étude montre que la dépression, dans sa forme typique, n'est pas rare à l'adolescence mais qu'elle est aussi souvent méconnue. C'est dire la nécessité de là rechercher devant tout problème psychopathologique survenant à l'adolescence. C'est dire aussi l'intérêt des études épidémiologiques visant à préciser la phénoménologie de la dépression à cet âge de la vie.

Zolpidem is a GABA (A) agonist, which is indicated for the short-term management of insomnia. Recent research provide evidence suggesting that zolpidem produces spatial working memory (WM) deficits and dependence; however, the underlying mechanisms of these effects are unknown. Since the auditory N400 component of event-related potentials (ERPS) is considered as an index of memory use of context processing, the present study focused on N400 waveform of ERPs elicited during a WM task in a case suffering from zolpidem dependence. The patterns of N400 waveform of this case were compared to the patterns obtained from healthy controls. This comparison revealed that zolpidem dependence is accompanied by reduced amplitudes located at posterior brain areas and diffuse prolongation of N400. These findings may indicate that zolpidem dependence manifests alterations with regard to the memory use of context processing, involving or affecting a wide-ranging network of the brain's structures.

The aim of this study was to investigate relationships and differences between 2 scales assessing subject’s adaptative functioning. On the one hand, Axis V in the revised version of the Third Edition of the Diagnostic and Statistical Manual of Mental Disorders (DSM III-R) includes specific symptoms for each level of functioning described. On the other hand, the Psychosocial Aptitude Rating Scale (PARS) does not include clinical symptoms. A total of 78 psychiatric inpatients were rated independently with the 2 scales. The 2 ratings were correlated for the whole sample and for each diagnostic category. Diagnosis (Axis 1) explained 63.9% of the variance in the Global Assessment Functioning (GAF) Scale (Axis V) and only 23.8% in the Psychosocial Aptitude Rating Scale (PARS). Axis V is then made redundant by Axis 1 which is contrary to the aim of a multi-axial system. PARS describes aspects of the disorder that are distinct from clinical symptoms. These occupational and social aspects may be of great interest for a better overall evaluation of severity of mental disorders.

The prolactin, cortisol and growth hormone (GH) responses to intravenous administration of 25 mg clomipramine (CMI) were studied in young male psychotic patients who had never received neuroleptics and suffered from schizophrenia (13 patients), delusional disorder (three patients) or schizoaffective disorder (one patient). The test was repeated after 1 month in 16 patients who were hospitalized and treated with haloperidol in doses appropriate for best clinical response (range: 7.5–40 mg daily). Symptomatology was assessed by the Brief Psychiatric Rating Scale (BPRS). There was no association of the side effects caused by the administration of CMI (nausea and emesis) to the GH responses. The side effects appeared significantly less in the after treatment trials. Treatment with haloperidol did not influence the response patterns of the three hormones. An indication that high haloperidol doses may inhibit the prolactin response to CMI was obtained when the data were compared between low (7.5–10 mg/day, mean 9.7) and high (15–40 mg/day, mean 22.0) dose subgroups. Significant positive correlations were found between the prolactin and cortisol responses to CMI in the drug-free state, and the scores in the positive symptoms subscale of the BPRS. The degree of improvement did not correlate to any of the hormonal data.

There has been continued interest in the nature of the association between affective disorders and alcoholism. This may be due to the fact that during the course of alcoholism 30-70% of the patients suffer from mood disturbances meeting the criteria for severe depression, especially in periods of excessive alcohol consumption or during withdrawal. In the course of a prospective multidimensional study, Tiibinger Alkoholismusprojekt (T-ALK Study), symptoms of depression in 31 male alcohol-dependent patients (RDC diagnosis) were assessed using the Beck Depression Inventory (BDI) and the Freiburger Personlichkeitsinventar (FPI-R) at the beginning and at the end of a 6-week inpatient treatment program. There was only a moderate degree of depression (BDI) in 20% of the patients which decreased significantly during the psychotherapeutic treatment under conditions of confirmed abstinence. No correlations between the degree of depression and neuropsychological impairment were found. Although the “satisfaction with life” (FPI-R factor) was still impaired in a third of the patients at the end of that short-term treatment period, our results do not indicate a general nosological overlap between alcoholism and affective disorders in males.

Authors recently have suggested that family enrneshment is not synonymous with high levels of closeness or cohesion. A model proposed by Green and Werner clarifies the cohesion-enmeshment domain by distinguishing between closeness-caregiving and intrusiveness as separate relationship processes. This paper examines the cross-cultural applicability of this perspective through a study of 61 married couples in France. The French version of the California Inventory for Family Assessment (CIFA), a self-report measure designed to assess clinically relevant marital dimensions, was employed. In general, spouses' reports of their marital process demonstrated high internal consistency reliabilities. Factor analysis showed meaningful factor structures distinguishing closeness-caregiving and intrusiveness, as predicted, as well as openness of communication. Significant correlations were obtained between CIFA scales and scores on the Marital Adjustment Test. These results are similar for French and American couples. Research implications for studying relationships among French-speaking couples are underlined.

Part II of this paper contains some general considerations on tricyclic antidepressant (TCA) monitoring. Long-term assessment of TCA plasma levels is advised by the few existent studies, although each of these focusses on different aspects. Cardiovascular and central nervous system toxicity is reviewed as well as pharmacokinetics and the importance of protein binding. Some consideration is also given to their use in elderly patients. The authors conclude that although available data support its usefulness in many situations, routine measurement of TCA levels is not warranted.

In an open prospective study, the stabilizing effects of carbamazepine on agitation, impulsivity and affective lability were investigated in 15 severely demented patients, who had failed to respond to prior neuroleptic medication. Because of leukopenia and allergic reactions in two subjects, carbamazepine treatment was discontinued. In nine patients a clinically relevant improvement was observed.

In this study we tested whether initial clinical change (ICC) and initial subjective response (ISR) predict the outcome of antidepressant pharmacotherapy and whether ICC and ISR as predictors reflect specific pharmacological actions or a placebo effect. Forty patients with major depression were treated with three different antidepressants for 4 weeks. Overall clinical change and final subjective response were taken as outcome criteria. The patients were randomly assigned to two subgroups in a double-blind design. Initially, one group received the active drug and the other placebo. Afterwards, all patients were given the active drug. Significant correlations were found between ICC and ISR and at least one of the outcome criteria in the total sample and in each subgroup. The findings show that ICC and ISR may be significant predictors of outcome. The predictive value of ICC and ISR is not due to initial pharmacological effects, but to non-specific treatment factors.

The aim of this study was to identify possible peripheral biological markers (both lipidic and hormonal) which can be easily used for the early detection of parasuicidal behaviour and to propose a predictive biological model of such behaviour. A case-control analytical study was undertaken at least 3 months after attempted suicide. Study was made of 128 patients who presented at the University General Hospital of Oviedo (Spain) with signs of self-intoxication. Lipidic and hormonal profiles were measured under basal conditions and comparison was made with a control group of healthy volunteer donors obtained from the Oviedo General Hospital blood bank. A discriminant analysis was later made with the aim of establishing a predictive biological model. This included the following variables: cholesterol, HDL-C, LDL-C and cortisol. Sensitivity and specificity were 62.5% and 65.6%, respectively. Replication and improvement of this model, through other prospective studies, could lead to the use of serum cholesterol and cortisol levels as inexpensive and readily available markers of suicide risk.

The authors examined whether substance abusers have more maladaptive thinking patterns than controls. The Dysfunctional Attitude Scale (DAS) was administered to 38 substance abusers and 30 healthy age-matched controls. Nineteen substance abusers (50%) were depressed and 19 were non-depressed. Both depressed and non-depressed substance abusers had significantly higher DAS scores than controls. Depressed substance abusers had higher scores than non-depressed substance abusers. There was no correlation between age, sex, age of onset or the nature of the substance used and DAS scores in either group of patients. There was a trend for an elevation of DAS scores with chronicity of dependence among depressed substance abusers. The results support the hypothesis of the presence of maladaptive thinking patterns among substance abusers, and suggest a need for further exploration of their cognitive styles and of the use of cognitive therapy in the treatment of addiction.

Staff members of psychiatric facilities are at high risk of secondhand smoking. Smoking exposure was assessed in 41 nonsmoking employees of a psychiatry department before and after a ban. Subjective exposure measures decreased in 76% of the subjects. Salivary cotinine decreased in the subsample of seven subjects with high pre-ban levels (32 ±8 vs 40 ± 17 ng/ml, p = .045).
