Cognitive deficits and immune system dysregulation are core features of psychotic disorders. Among inflammatory markers, interleukin-6 (IL-6) and tumour necrosis factor-alpha (TNF-α) have been linked to both psychosis pathophysiology and related cognitive impairments.
Methods
We investigated associations among IL-6, TNF-α, and neurocognitive performance in 107 participants: individuals at clinical high risk for psychosis (CHR-P, n = 35), first-episode psychosis (FEP, n = 39), and healthy controls (HC, n = 33). Assessments included memory, processing speed, executive function, and social cognition. Cytokines were measured from fasting serum samples. Analyses included ANOVA, correlations, and multivariate regressions controlling for age, sex, IQ, group, and symptom severity.
Results
TNF-α levels were significantly elevated in FEP compared to CHR-P (p = 0.0251); IL-6 differences were non-significant. FEP showed poorer performance in multiple cognitive domains, especially social cognition. CHR-P individuals exhibited intermediate profiles between FEP and HC in cognition. In adjusted regression models, IL-6 was significantly associated with undermentalization on the MASC task (β = 0.28, p = 0.0337) and showed a trend-level association with slower processing speed (β = 0.98, p = 0.075). TNF-α levels predicted poorer facial emotion recognition (β = −1.37, p = 0.0022). IQ and group were significant covariates in most models.
Conclusions
Our findings suggest that peripheral inflammation, particularly IL-6 and TNF-α, may selectively impact social cognitive functioning in early psychosis. Though modest, these associations highlight potential inflammatory contributions to functional impairment and support further investigation of immunological targets in early intervention.
The number of published systematic reviews has increased over the last years, with a non-negligible proportion displaying methodological concerns. We aimed to develop and evaluate a tool to assess the reported methodological quality of medical systematic reviews. The developed tool (ReMarQ) consists of 26 dichotomous items. We applied an item response theory model to assess the difficulty and discrimination of the items and decision tree models to identify those items more capable of identifying systematic reviews with higher reported methodological quality. ReMarQ was applied to a representative sample of medical systematic reviews (excluding those published in the Cochrane Database of Systematic Reviews) to describe their methodological quality and identify associated factors. We assessed 400 systematic reviews published between 2010 and 2020, of which 196 (49.0%) included meta-analysis. The most discriminative items were (i) conducting a risk of bias assessment, (ii) having a published protocol and (iii) reporting methods for solving disagreements. More recent systematic reviews (adjusted yearly RR=1.03; 95%CI=1.02 −1.04, p<0.001) and those with meta-analysis (adjusted RR=1.34; 95%CI=1.25 −1.43, p<0.001) were associated with higher reported methodological quality. Such an association was not observed with the journal impact factor. The items most frequently fulfilled were (i) reporting search dates, (ii) reporting bibliographic sources and (iii) searching multiple electronic bibliographic databases. ReMarQ, consisting of dichotomous items and whose application does not require subject content expertise, may be important (i) in supporting an efficient quality assessment of systematic reviews and (ii) as the basis of automated processes to support that assessment.
Few studies have examined the long-term outcomes of first-episode psychosis (FEP) among patients beyond symptomatic and functional remission. This study aimed to broaden the scope of outcome indicators by examining the relationships between 12 outcomes of FEP patients at 20.9 years after their initial diagnosis.
Methods
At follow-up, 220 out of 550 original patients underwent a new assessment. Twelve outcomes were assessed via semistructured interviews and complementary scales: symptom severity, functional impairment, personal recovery, social disadvantage, physical health, number of suicide attempts, number of episodes, current drug use, dose-years of antipsychotics (DYAps), cognitive impairment, motor abnormalities, and DSM-5 final diagnosis. The relationships between these outcome measures were investigated using Spearman’s correlation analysis and exploratory factor analysis, while the specific connections between outcomes were ascertained using network analysis.
Results
The outcomes were significantly correlated; specifically, symptom severity, functioning, and personal recovery showed the strongest correlations. Exploratory factor analysis of the 12 outcomes revealed two factors, with 11 of the 12 outcomes loading on the first factor. Network analysis revealed that symptom severity, functioning, social disadvantage, diagnosis, cognitive impairment, DYAps, and number of episodes were the most interconnected outcomes.
Conclusion
Network analysis provided new insights into the heterogeneity between outcomes among patients with FEP. By considering outcomes beyond symptom severity, the rich net of interconnections elucidated herein can facilitate the development of interventions that target potentially modifiable outcomes and generalize their impact on the most interconnected outcomes.
For over two decades, political communication research has hailed the potentially reinvigorating effect of social media on democracy. Social media was expected to provide new opportunities for people to learn about politics and public affairs, and to participate politically. Building on two systematic literature reviews on social media, and its effects on political participation and knowledge (2000–2020), and introducing empirical evidence drawing on four original US survey data that expands for over a decade (2009–2020), this Element contends that social media has only partially fulfilled this tenet, producing a Social Media Democracy Mirage. That is, social media have led to a socio-political paradox in which people are more participatory than ever, yet not necessarily more informed.
First-episode psychotic disorders comprise a heterogeneous phenotype with a complex etiology involving numerous common small-effect genetic variations and a wide range of environmental exposures. We examined whether a family of schizophrenia spectrum disorder (FH-Sz) interacts with an environmental risk score (ERS-Sz) regarding the outcome of patients with non-affective first episode psychosis (NAFEP).
Methods:
We included 288 patients with NAFEP who were evaluated after discharge from an intensive 2-year program. We evaluated three outcome measures: symptomatic remission, psychosocial functioning, and personal recovery. We analyzed the main and joint associations of a FH-Sz and the ERS-Sz on the outcomes by using the relative excess risk due to interaction (RERI) approach.
Results:
A FH-Sz showed a significant association with poor symptomatic remission and psychosocial functioning outcomes, although there was no significant interaction between a FH-Sz and the ERS-Sz on these outcomes. The ERS-Sz did not show a significant association with poor symptomatic remission and psychosocial functioning outcomes, even though the magnitude of the interaction between ERS-Sz and FH-Sz with the later outcome was moderate (RERI = 6.89, 95% confidence interval −16.03 to 29.81). There was no association between a FH-Sz and the ERS-Sz and personal recovery.
Conclusions:
Our results provide further empirical support regarding the contribution of FH-Sz to poor symptomatic remission and poor psychosocial functioning outcomes in patients with NAFEP.
A large body of research states that cognitive impairment in schizophrenia is static. Nevertheless, most previous studies lack a control group or have small study samples or short follow-up periods.
Method
We aimed to address these limitations by studying a large epidemiological cohort of patients with first-episode schizophrenia spectrum disorders and a comparable control sample for a 10-year period.
Results
Our results support the generalized stability of cognitive functions in schizophrenia spectrum disorders considering the entire group. However, the existence of a subgroup of patients characterized by deteriorating cognition and worse long-term clinical outcomes must be noted. Nevertheless, it was not possible to identify concomitant factors or predictors of deterioration (all Ps > 0.05).
Conclusions
Cognitive functions in schizophrenia spectrum disorder are stable; however, a subgroup of subjects that deteriorate can be characterized.
In this work, the stopping power of a partially ionized helium plasma due to its free and bound electrons is analyzed for an electron temperature and density in which local thermal equilibrium (LTE) or non-local thermal equilibrium (NLTE) regimes can be possible. In particular by means of collisional-radiative models, the average ionization of the plasma as well as the abundances of different helium species (HeI, HeII, and HeIII) are analyzed in both LTE and NLTE thermodynamic states. The influence of this ionization and of the different ion abundances on the stopping power of the helium plasma is shown to be quite significant. Finally, our theoretical model is compared with experimental results on slowing down of swift argon ions in helium plasma.
The Endangered Cuvier's gazelle Gazella cuvieri is an endemic ungulate of north-western Africa. Information on the species has been based primarily on non-systematic surveys, and the corresponding status estimates are of unknown quality. We evaluate the effectiveness and efficiency of two field methods for systematic surveys of populations of Cuvier's gazelle in arid environments: distance sampling (based on sightings) and sampling indirect sign (tracks and scats). The work was carried out in the north-western Sahara Desert, in Morocco, where what is possibly the largest population of Cuvier's gazelle persists. A logistically viable survey was conducted over a total area of c. 20.000 km2 in 10 expeditions during 2011–2014. A total of 67 sites were surveyed, with 194 walking surveys (2,169 km in total). Gazelle signs were detected at 50 sites, and gazelles were sighted at 21 sites (61 individuals). We found a relationship between sightings and abundance indices based on indirect sign, which could be useful for population monitoring or ecological studies. Additionally, the data could be used in occupancy modelling. Density estimates based on distance sampling required considerable effort; however, it is possible to survey large areas during relatively short campaigns, and this proved to be the most useful approach to obtain data on the demographic structure of the population.
To assess classical and non-classical metabolic risk biomarkers in prepubertal children with different levels of cardiorespiratory fitness (CRF).
Design
CRF was assessed by the 20 m shuttle run test. To estimate physical activity, participants were observed while engaged in an after-school programme. Additionally, a short test based on a validated questionnaire was used to obtain information about physical activity practice and sedentary habits. Anthropometric parameters, blood pressure, and classical and non-traditional metabolic risk biomarkers – plasma lipid profile, glucose and insulin, homeostasis model assessment–insulin resistance index (HOMA-IR), plasma uric acid, transaminases and C-reactive protein (CRP) – were measured.
Setting
The study was conducted in local elementary schools in Córdoba, Spain.
Subjects
One hundred and forty-one healthy children (eighty-eight boys, fifty-three girls) aged 7–12 years, in Tanner stage I, were recruited. They were divided into two groups after they performed the 20 m shuttle run test: equal or higher cardiovascular fitness (EHCF) group and low cardiovascular fitness (LCF) group.
Results
The LCF group displayed significantly higher TAG (P = 0·004) and lower HDL cholesterol levels (P = 0·001), as well as significantly lower values for the non-traditional lipid marker apo-A1 (P = 0·001) compared with the EHCF group. The LCF children displayed higher plasma glucose (P = 0·003) and insulin levels, higher HOMA-IR scores (P < 0·001) and higher plasma uric acid and CRP levels (P < 0·05). After adjustment for BMI, age and sex, no statistically significant differences were found between groups for the biomarkers analysed.
Conclusions
The study provides new information to understand the role not only of weight status but also of the level of CRF on the metabolic health profile of prepubertal children.
Thus far, little has been written concerning echocardiographic identification of the oblique vein of the left atrium, or Marshall’s vein. There is much discussion, nonetheless, on the potential significance of the vein, or its ligamentous remnant, as an arrhythmic substrate. We describe here four patients in whom transthoracic echocardiography revealed a venous structure protruding within the cavity of the left atrium. We discuss the possibility that these structures represent Marshall’s vein, albeit probably as part of a persistent left superior caval vein.
It has been traditionally assumed that medial temporal lobe (MTL) is not required for working memory (WM). However, animal lesion and electrophysiological studies and human neuropsychological and neuroimaging studies have provided increasing evidences of a critical involvement of MTL in WM. Based on previous findings, the central aim of this study was to investigate the contribution of the MTL to verbal WM encoding. Here, we used magnetoencephalography (MEG) to compare the patterns of MTL activation of 9 epilepsy patients suffering from left hippocampal sclerosis with those of 10 healthy matched controls while they performed a verbal WM task. MEG recordings allow detailed tracking of the time course of MTL activation. We observed impaired WM performance associated with changes in the dynamics of MTL activity in epilepsy patients. Specifically, whereas patients showed decreased activity in damaged MTL, activity in the contralateral MTL was enhanced, an effect that became significant in the 600- to 700-ms interval after stimulus presentation. These findings strongly support the crucial contribution of MTL to verbal WM encoding and provide compelling evidence for the proposal that MTL contributes to both episodic memory and WM. Whether this pattern is signaling reorganization or a normal use of a damaged structure is discussed. (JINS, 2009, 15, 536–546.)
A study was carried out to analyse the production and composition of milk from Malagueña goats of different genotypes for the content of αs1-casein. Twenty goats were used, half of them with a high (HG) and half with a low (LG) genetic capability for αs1-casein synthesis. Among the ten HG goats, seven had BB and three had AB genotypes, whereas there were seven EF and three FF genotypes among the LG goats. The goats were fed a diet differing in crude protein content, 136 and 177 g/kg dry matter for diet 1 (D1) and diet 2 (D2), respectively. For each genotype group, a trial was conducted, with each of the two diets in a two-period balanced change-over design, in which the milk production and composition were analysed. The genotype was found to determine milk composition, with that produced by the HG goats being higher in total solids, protein, αs1-casein, αs2-casein and fat. Analysis of milk production values and, especially, those of the yields of the different components, revealed an interaction between the factors. HG goats that consumed D1 were more productive than LG goats, but this ceased to be so when the D1 diet was replaced by D2. However, LG goats, although less productive than HG ones when the D1 diet was consumed, showed increased productive capacity when D1 was replaced by D2. The main difference for the fatty acid profile of the dairy fat is that the milk fat produced by the HG goats contained higher levels of short and medium-chain fatty acids. From these results, it is concluded that, in terms of favouring production, the advantage of replacing animals with a low capacity for αs1-casein synthesis with those of a high capacity seems to depend on the protein level in the diet. Similarly, the advantage of changing to a diet of higher protein content depended on the animal's genotype.
This paper presents molecular dynamics simulations of shear-coupled migration of tilt boundaries pinned by triple junctions in a simple model structure of columnar grains of different sizes. Simulations are for copper at 300 K. The phenomenon is of interest as a possible explanation of the Hall-Petch relationship breakdown in nano-grained polycrystals deformed at high or moderate strain rate and low-temperature.
The elastic deformation contribution can significantly affect the measurement of the strain-rate sensitivity (SRS) of the plastic flow stress by indentation methods. In this paper, the effect of such elastic contribution is critically analyzed using an extension of a previous treatment developed by the authors for the elastic effects on the indentation of strain-hardening materials [J. Alkorta et al., J. Mater. Res.20, 432 (2005)]. The analytical model is calibrated and validated through finite element calculations. The results show that when the elastic contribution to the total deformation is not negligible then the measured SRS is significantly lower than the real one. A satisfactory correction factor for the apparent SRS exponent is proposed based on parameters directly accessible to instrumented indentation test results.
Slope Water anticyclonic Oceanic eDDIES (SWODDIES) are typical mesoscale features of open-ocean waters of the southern Bay of Biscay which usually develop in winter by shedding from the seasonal poleward current flowing along the northern Spanish slope. These eddies have been intensively studied from the physical perspective. However, their effect on the distribution of biological properties and on the functioning of the pelagic ecosystem has not been assessed so far. To this aim, a sea-truth, multidisciplinary and comprehensive study of a SWODDY was carried out in summer 1998. The eddy, radius of ≈50 km, was initially centred at 45·5°N 6·0°W, being characterized by a relatively homogeneous core of water in the centre of the eddy extending from 80 to about 200 dbar. In the central region of the core, temperature (12·55–12·75°C) and salinity (≈35·70) values were higher than outside the eddy. The optical properties of the eddy also differed from those of the surrounding waters. A distinct biological signature was found associated with the eddy. Depth-integrated chlorophyll-a concentrations were 25% higher at the eddy centre where upward doming of the seasonal pycnocline (up to 30 dbar) occurred. Enhanced phytoplankton biomass was related to a higher contribution of >10 μm cells, mainly represented by diatoms and chrysophyceans. Phytoplankton and mesozooplankton species composition in and outside the eddy differed significantly reflecting the coastal origin of the water parcel trapped by the eddy. The sharp modification of the planktonic community composition, biomass and associated size-structure caused by slope water oceanic eddies are likely to exert a significant effect upon the upper trophic levels of the pelagic ecosystem of the southern Bay of Biscay.
The full paper describes an environment for the generation of non-deterministic taggers, currently used for the development of a Spanish lexicon. In relation to previous approaches, our system includes the use of verification tools in order to assure the robustness of the generated taggers. A wide variety of user defined criteria can be applied for checking the exact properties of the system.
