Research involvement is good for both patients and clinicians. In healthcare organisations that are research active, not only are mortality outcomes better, but clinicians are happier and retention rates better. In this chapter we consider how early career clinicians can involve themselves in research, for the benefit of your clinical practice and your patients. It is important to foster early exposure to research, which also makes further involvement in research a less intimidating venture. The only warning attached to that is that once you’ve had a taste of research involvement, and seen your first publication in print, you may find it hard to leave it behind! Establishing research skills early in one’s career can have advantages; firstly, it encourages critical thinking on how to approach any patient, their presenting difficulties and the selection of appropriate interventions. Further, being able to appraise the expanding and complex (and often contradictory) evidence base is a vital skill any clinician will utilise throughout their career. Finally, it is, or can be, enormous fun!
This chapter explores the critical importance of child and adolescent mental health in understanding and mitigating mental illness across the life course. Because the majority of mental disorders emerge during childhood or adolescence, early mental health interventions are key to prevent long-term health burden. We emphasise the need for a developmental perspective in mental health research, highlighting the challenges and opportunities in studying both typical and atypical development, addressing diagnostic comorbidity, and evaluating environmental influences on mental health outcomes. We discuss four examples to illustrate the breadth of research in child and adolescent mental health: (1) conceptualising psychopathology across the lifespan, (2) establishing valid measures of childhood maltreatment to assess its impact on mental health, (3) testing the mechanisms that might explain why maltreatment contributes to the risk of psychopathology, and (4) optimising psychiatric crisis care for adolescents in the UK. By examining these critical issues, the chapter outlines how advances in child and adolescent mental health research can lead to innovative strategies for preventing and managing mental health problems, with the potential to improve wellbeing throughout the life course.
Publishing a peer-reviewed paper is an ambition for all entering academic careers, and a necessity for those remaining in it. It can feel a daunting, complex, and long process, but there is an art and skill to successfully publishing that can be learned. To better meet the growing demand and complexity of clinical need, there is a broad international trend towards greater integration of various elements of health- and social care. However, there has been a lack of research aimed at understanding how healthcare providers have experienced these changes, including facilitators and inhibitors of integration.
Aims
This study set out to generate new understandings of this from three UK staffing ‘levels’: ‘micro’ frontline workers, a ‘meso’ level of those leading a healthcare organisation and a ‘macro’ level of commissioners.
Method
Using Rogers’ Diffusion of Innovation framework, qualitative analysis of individual interviews from provider staff perceptions was undertaken at these three levels (total N = 33) in London.
Results
English legislation and policy captured the need for change, but fail to describe problems or concerns of staff. There is little guidance that might facilitate learning. Staff identity, effective leadership and culture were considered critical in implementing effective integration, yet are often forgotten or ignored, compounded by an overall lack of organisational communication and learning. Cultural gains from integration with social care have largely been overlooked, but show promising opportunities in enhancing care delivery and experience.
Conclusions
Findings are mixed insofar as staff generally support the drivers for greater integration, but their concerns, and means for measuring change, have largely been ignored, limiting learning and optimisation of implementation. There is a need to emphasise the importance of culture and leadership in integrated care, and the benefits from closer working with social care.
Recent changes to US research funding are having far-reaching consequences that imperil the integrity of science and the provision of care to vulnerable populations. Robust regulatory scrutiny is an unavoidable and necessary part of professional life for public sector professionals. Inspection and investigation can lead to poor mental health for individuals already working under pressure owing to increased workload and anticipation of poor outcomes. Although good regulation maintains standards and provides accountability to government and the public, regulators must face their obligation to understand the wider impact of their practices on the mental health of those they evaluate. This article discusses how regulation affects public sector culture and the potential risks and negative impact of regulatory practices and highlights how clinicians, working in occupational practice, are well placed to recognise ‘regulatory stress’ among public sector workers and offer vital support, guidance and advocacy.
